New Patient Registration

If you wish to register you will need to follow our 2-step process by checking you live within the boundary area and completing one of our online forms below.

When you have completed all of the details, click on the "Submit Form" button to mail your form to us. We will send you a message once you are registered.

Alternatively, you can collect an appropriate registration pack from reception and bring it to the Surgery any weekday between 10am to 4:30pm.

Please be aware that you will be asked to show 2 forms of identification, one being a photographic identification and the other with details of your home address.

Change of Details

Please inform us if you've moved or updated personal details so we can keep accurate records. This can include:

  • Your Address
  • Name
  • Telephone Number
  • Mobile Number

It is essential that our records are kept up to date in order to provide you with the best service. You can change your details via our secure online form below.

Update My Details

Temporary Services

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Run out of medication and away from home

If you have run out of your repeat medication whilst, on holiday in our town, you do not need to register as a temporary resident.

Please contact your own GP practice who can send your repeat medication electronically to a local pharmacy (you will need to know the name and address of the pharmacy).

Your Named GP

All Patients registered with the surgery will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the surgery. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
